A lifting task at an open-cut coal mine in New South Wales has highlighted the importance of clear communication and pre-lift checks after an engine unexpectedly slewed forward while being removed from a haul truck.
According to the NSW Resources Regulator, the incident occurred while workers were removing the engine from a 793-haul truck using a crane.
During the lift, the engine snagged on a hose fitting, causing it to move unevenly. As the load shifted, the engine slewed forward, prompting two workers nearby to take evasive action.
In an attempt to stabilise the load, the crane operator tried to lower the engine to relieve pressure on the hose. However, the hose disconnected during the process, causing the engine to slew approximately two metres forward.
Following the incident, the regulator is reminding operators that lifting work requires careful coordination between crews and crane operators, particularly when removing large or complex equipment.
Before moving a load, workers should confirm that all pipes, hoses and cables have been fully disconnected. Any remaining connections can catch or snag during a lift, increasing the risk of sudden movement.
The regulator said risk assessments and procedures should also consider the potential for unexpected load movement and outline clear controls for workers to follow.
Supervisors play a key role in managing these risks, and the regulator recommends that they clearly explain safe spotter positions and ensure workers understand when to use tag lines or other methods to maintain safe separation from suspended loads.
