The importance of effective defect reporting and inspection processes has been reinforced after a haul truck’s steering wheel detached while the vehicle was travelling at an open-cut coal mine.
According to the NSW Resources Regulator, the loaded haul truck was travelling at about 41km/h when the steering wheel became loose and popped off the steering column spindle.
The operator was able to safely stop the truck using the service brake, bringing the vehicle to a halt in the centre of the haul road.
The regulator said the truck’s steering column had been replaced several months earlier after a steering wheel issue had been reported.
Three days before the incident, another steering-related concern was entered into the mine’s defect reporting system under the category of “other” rather than “steering”. Because of the incorrect classification, the truck was not automatically stood down in line with the site’s safety controls.
In response, the regulator reminded mine operators that defect reporting systems and pre-start inspections must be used effectively and in accordance with site procedures to ensure mobile plant remains safe to operate.
It also said reports of defects or operator concerns should trigger thorough inspections and testing before equipment is returned to service, particularly where safety-critical systems are involved.
The regulator said steering systems should be inspected, maintained and tested in accordance with manufacturers’ recommendations, with all maintenance work verified before plant is returned to operation.
